New patients must register by clicking on “Sign Up” under the blue bar of page links at the top right of the scheduling screen. This link opens a new window.

Please make sure to include all contact information (if your primary phone is a cell phone, please enter this in both "Phone1" and "Mobile").  This step must be completed so that we have your contact info. 

Once you are signed up, you can easily schedule yourself for future appointments by signing in, chosing a day, then selecting a service type and then clicking on an AVAILABLE time. 

Select a service type by clicking to the right on “ + Click here for a list of Appointments”.  If you’ve been to the Belmar Chiropractic Clinic before select “Regular Patient Visit”.  If you are a NEW PATIENT, please select, “NEW Patient Initial Exam”.

Belmar Chiropractic Clinic is currently contracted/in-network with Aetna and Cofinity (formerly Sloan’s Lake) Insurance companies.  This DOES NOT MEAN that we won’t bill for other health insurances. 

If you do have another insurance plan, you may be required to pay "Out-of-Pocket" until a deductible is met.  We will check your Out-of-Network Chiropractic benefits for you if you simply FAX the front and back of your insurance card to us at 303-233-1084 and supply your date of birth as well.

NEW Patient Initial Exams are scheduled for 1 hour.  This time is budgeted for filling out New Patient Forms.  If you would like to save time by filling out these forms in advance, download our New Patient Forms print the PDF files.  You will still need to sign HIPPA and Consent forms when you come in.
